Health Service Board Election

 

The Department of Elections will conduct the 2006 Health Service Board Election. This election will choose one member for the Health Service Board for one full term ending in 2011.

Since there were only two candidates participating in the process, the withdrawal by Mr. Crandall will result in the current Health Service Board incumbent, Karen Breslin, serving an additional term on the Health Service Board beginning May 16, 2006.

Voting period

The voting period will end at 5:00 p.m.,Friday May 26, 2006.

Ballot delivery and return:

  • Envelopes containing the ballot and Voter Information Pamphlet will be mailed to all active and retired members no later than May 3, 2005
  • Ballots should be marked, placed inside the Return envelope and sealed

All voted ballots must be received by the Department of Elections no later than 5:00 PM on May 22, 2006.
Any ballots received after this deadline will not be counted.

Request for replacement ballot:

To request a replacement ballot, members are required to fill out the Perjury Notice form. This form must be filled out, signed, and returned to the Department of Elections. You may return the form by mail or by fax at (415) 554-7257. Once received, the Department will mail you a new ballot.

Note: Please submit your request on or before May 28, 2006 to give us time to mail your ballot and for you to return it to us on time.

After May 28, 2006, all requests must be made in person at our office.
