Forms with asymbol must be signed with pen to paper (digital signatures are not allowed). Individuals who are unable to sign may make a mark witnessed by a person 18 years of age or older, or use their certified signature stamp.
Voter Forms and Self-Help Tools
Accessible Vote-by-Mail System
Voters with disabilities and military and overseas voters may use the Accessible Vote-by-Mail (AVBM) system to access and mark their ballots from internet-connected devices, using personal assistive technology, such as screen readers, head-pointers, or sip and puffs.
California Online Voter Registration Application
Complete this online form to pre-register, register, or re-register to vote. (Those with California driver's licenses or state identification cards do not need to print or mail the online form.)
Emergency Ballot Service Request Form (PDF)
Complete this form if you are unable to travel to a polling place or voting center because of illness, disability, or confinement to authorize another person to deliver your ballot to you.
Ballot Pickup Authorization Form (PDF)
Complete this form to authorize your spouse, child, parent, grandparent, grandchild, sibling, or a person residing in your household to pick up a vote-by-mail ballot and deliver it to you.

Notification of Deceased Voter Form
Complete this form to report that a San Francisco voter has passed away (Department staff will check death records and attempt to notify the voter before canceling the voter's registration).
Request to Cancel Voter Registration
Complete this digital form to cancel your voter registration if, for example, you move out of San Francisco or out of California. The form must be printed and signed.
Request to Receive Translated Materials
Use this form to add a language preference to your voter registration file and receive translated election materials in your preferred language.
Request to Stop or Resume Receiving the Voter Information Pamphlet and Sample Ballot or Voter Guide by Mail
Login to the Voter Portal and go to the "Stop/resume the mailing of my voter guides" tab to request to stop receiving mailed copies of the local Voter Information Pamphlet and statewide Voter Information Guide, or request to resume these mailings.
Third-Party Voter Registration Notification Form
Complete this form to notify the Department of Elections that one or more recipients of official election mail no longer resides at a certain address.
Ballot Return Form (PDF)
Complete this form and follow its detailed instructions to submit a vote-by-mail ballot using two regular envelopes (this is only necessary if you cannot use your official ballot return envelope).

Complete this form to assign the right to publish a ballot argument to another person or organization otherwise qualified under SFMEC 545 or 530 but not selected via priority or lot.

Complete this form, along with Control Sheet A (and Control Sheet B if applicable), to submit a ballot argument which includes a reference to a person or organization who is not an author.
Ballot Argument Control Sheet A (PDF)
Complete this form to submit a ballot argument authored by any number of people or organizations. If the argument has more than one author, submit Control Sheet B as well.
Campaign and Candidates Appointment Form
Complete this form to request an appointment with the Department of Elections' Campaign Services Division. Note that no such appointments are available on filing deadline dates.
